Alzheimer’s & Caregiver’s Support

Our community centers provide caregiver support groups, relying on group processes to assist caregivers in developing competencies and coping strategies related to their caregiving experience. The goals of these services are to create a comfortable setting to offer emotional support, encourage information sharing, and promote skills development. Participants have an opportunity to connect with the community and other caregivers. Support is offered to any caregiver regardless of their participant’s cognitive function.

Additionally, at our community center in Ward 4, Washington DC, we acknowledge the profound impact of Alzheimer's disease, particularly the memory loss and daily challenges it presents. Congregate Meals

Our community dining sites in Ward 4 offer enriching experiences for individuals aged 60 and older. Each location is thoughtfully designed to provide warm, nourishing meals in a welcoming social environment. Counseling and Social Worker

Counseling Services

Experience the difference of compassionate care with a dedicated team of licensed social workers, providing expert counseling services designed to support you and your family on your journey in Ward 4. These services are available for delivery over the phone, in-person, or via electronic communication. We offer individual counseling for caregivers and psycho-social wellness interventions for our participants. The goal is to provide emotional support, mental health intervention, and improve coping skills. Our counseling services are short-term and solution-focused. They may include, but are not limited to, identifying the caregiver’s and the participant's personal strengths and abilities, managing both short-term and long-term care decisions and planning, and developing strategies to better manage and cope with the caregiver role and the cognitive limitations of our participants.

Health Promotions

Selected community centers offer a health promotion program provided by registered nurses, physical fitness experts, and consultants in physical, occupational, nutritional, and speech therapy. These programs are designed to encourage healthy behaviors and lifestyles through health education, wellness screenings, nutrition, and physical fitness activities. The goal of these services is to provide a comprehensive range of structured programs, resources, and health-related activities to educate participants in Ward 4 on how to develop healthy lifestyles, preventing and/or managing diseases and delaying institutionalization.
